In our North Carolina location, we offer an array of outpatient Community Oriented Recovery and Empowerment (CORE) services that are designed to meet the various needs of children, adolescents, and adults. SFI serves Medicaid-funded youth up to 20 years old and state-funded youth up to age 17 years old, providing targeted support within the home setting. Our aim is to provide coordination of services and support clients’ efforts to maintain a healthy, thriving therapeutic journey with the least restrictive environments.

 

We coordinate:

Individual and Family Outpatient Counseling: Make as a dropdown option with an extension that highlights the following: A Comprehensive Clinical Assessment (CCA) is completed by a licensed therapist on our team to determine presenting problems, history of mental health and substance abuse challenges. Our program is designed to support adult and adolescent consumers in initiating recovery, teaching essential skills for maintaining sobriety and improving overall well-being.

Medication Management: Make as a dropdown option with an extension that highlights the following:  If a comprehensive clinical assessment (CCA) was completed, the medical professional will review it and seek further information as necessary. We provide careful review and ongoing assessment of current psychotropic medications. Our goal is to ensure appropriate medication use by identifying the need for modifications to optimize therapeutic outcomes while minimizing side effects.

Community Support Teams: CST is a community-based MH/SA rehabilitation service provided through a team approach to assist consumers in achieving rehabilitative and recovery goals. The team approach involves structured face-to-face scheduled therapeutic interventions to provide support and guidance in all areas of functioning including emotion, social, safety, housing, medical, health, education, vocational and legal.

Intensive In-Home Services and Case Management: Stabilizes the living arrangement, promotes reunification or prevent the utilization of out-of-home placement. In Home Therapy Services (IHTS) uses a combination of evidence based therapy services and coordination of care interventions provided in the home setting for individuals with complex clinical needs.  SFI provides focused family systems work and coordination to address the issues that are placing the child/adolescent at risk of out of home placement. With our case management approach, we help individuals and families navigate complex barriers to care, connecting them with doctors, resources (like food, housing, financial aid), and support to manage illnesses, achieve wellness goals, and improve overall well-being, often through coordination, advocacy, and education by professionals like nurses or social workers.

Keep the part on Disability services. We’ll just cut it down to a short sentence with a drop down with the short personal care, respite, and home and community support parts.

Developmental Disability Services: Our disability support ervices are tailored to meet the unique needs of each individual, promoting skill development, community integration, and personal growth.

Personal Care
Includes support, supervision and engaging participation with eating, bathing, dressing, personal hygiene and other activities of daily living.

Respite
Provides periodic relief for the family or primary caregiver.

Home & Community Supports
Provide instruction and assistance to enable the individual to acquire and maintain skills that will allow him/her to function with greater independence in the community.
